As an OT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program! In a world of shorter SNF stays, TYTH is a one‑of‑a‑kind program where the clinician works directly with patients in the SNF and home health, providing continuity of care for better outcomes. You will be primarily in the home health setting with dedicated time and caseload support for the SNF. You will have a localized coverage area around the SNF. Your schedule follows a traditional work week. Rivetus has been serving the geriatric Post‑Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. We provide continuity of care across Skilled Nursing, Home Health, and Outpatient settings.
