Job Description Brookdale Carriage Club is hiring an Infection Prevention and Associate Director of Clinical Services. This senior living community supports residents through Assisted Living, Independent Living, Memory Care, and Long Term Care, with over 400 residents. We seek an experienced RN with infection prevention expertise. Assists in maintaining systems and established policy and procedures. Assess the comprehensive nursing needs of each resident using acceptable long‑term care assessment software tools and according to state and federal time frames. Coordinate care plans for each resident and oversee day‑to‑day care to ensure care plans are followed. Work with other team members to establish appropriate level of care in accordance with Medicare/Medicaid reimbursement levels. Assist in ensuring comprehensive and accurate documentation by nursing service/direct personnel. Monitor, manage and provide education on Clinical Systems, programs and processes as assigned by the Director, Clinical Services; implement these systems and programs in accordance with current rules, regulations and guidelines that govern long‑term care. Monitor community Quality Indicators and survey reports and provide the Director, Clinical Services with recommendations for Success Planning, including conducting and scheduling in‑service training classes that provide instruction of clinical skills and required education topics. Plan, implement, evaluate and direct the facility’s Infection Prevention and Control Program. Apply scientific principles and methods in conducting surveillance, using approved definitions of infection and standard methodologies for case identification, data collection, and reporting. Plan and conduct educational programs based on prevention, trends, and outcomes. Prepare reports and presentations for the Quality Assurance Performance Improvement Committee, including Antibiotic Stewardship activities. Investigate and report outbreaks of infection and implement infection prevention interventions. Assist in implementing policies and procedures to support optimal associate compliance, infection prevention and resident safety in adherence to OSHA regulations and infection control standards. Assist in developing and implementing a Clinical Services organizational structure; determine staffing needs; recruit, select, orient nursing/direct care personnel; lead programs for staff development; develop work assignments, schedules duty hours, and delegate authority to supervisory/lead staff. Review complaints and grievances filed by personnel and/or residents; report and investigate allegations of abuse and/or misappropriation of resident property; report suspected or known violations of disclosure of resident protected health information. Assist with the coordination of ancillary services. Collect, coordinate, and submit MDS data to the state. Monitor medication and treatment schedules; work with attending physicians; provide direct care when required. Participate in maintaining the policy and procedures that govern the day‑to‑day operations of the Clinical Services Department; possess knowledge of state, federal, and OSHA regulations; implement policies and procedures to ensure compliance. Participate in developing and implementing safety standards and regulations for equipment, supplies, and resident care rooms; implement safe practices in accordance with state, federal, and OSHA regulations for resident safety. Monitor supply and equipment needs and budget; monitor Medicare/Medicaid reimbursements. Complete other duties as assigned. Qualifications Education and Experience Nursing degree from an accredited college or university; minimum 2‑4 years related experience and/or training; or equivalent combination. Long‑term care experience preferred. Certifications, Licenses, and Other Special Requirements Must have a valid and current state RN license issued by the appropriate state licensing agency. Knowledge and Skills Extensive knowledge of a functional discipline; ability to write routine reports and correspondence; ability to speak effectively before groups of customers or employees. Physical Demands and Working Conditions Standing, walking, sitting, use hands and fingers to handle or feel; reach with hands and arms; stoop, kneel, crouch, crawl; talk or hear; lift up to 25 pounds; vision; requires interaction with co‑workers, residents or vendors.
