RN/LPN – Adult Foster Care (Full-Time / Part-Time) Location: Massachusetts (Community-Based) Schedule: Full-Time or Part-Time Program: Adult Foster Care (AFC) Job Summary We are seeking a compassionate Registered Nurse (RN)/ Licensed Practical Nurse (LPN) to join our Adult Foster Care (AFC) team. This community-based role provides clinical oversight, member assessments, caregiver support, and health monitoring for adults and elders living safely at home. This position offers flexibility, autonomy, and meaningful one-on-one member interaction. Responsibilities Complete annual nursing assessments and clinical evaluations Develop and update AFC Plans of Care Monitor member health status and report changes to PCPs Conduct required home visits Review caregiver documentation and care logs Provide caregiver orientation and ongoing training Complete nursing progress notes and required reports Collaborate with Care Managers and interdisciplinary team Qualifications Active MA RN/LPN license CPR certified Minimum 2 years’ experience with elders or adults with disabilities Strong assessment and documentation skills Reliable transportation for community visits What We Offer Flexible scheduling Supportive team environment Mileage reimbursement Competitive pay Health Insurance Reimbursement, Paid Vacation Time, Sick Time, Paid Holidays Opportunity to make a meaningful community impact #J-18808-Ljbffr Sarah-Care-Adult-Foster-Care
