Social Worker/Case Manager

$35.91 - $47.29 per hour

Social Worker/Case Manager Location: North Branch, MN Position Overview Provide case management services to the aging and disabled population of Chisago County. Case managers develop service delivery plans that address environmental, social, safety, physical, and rehabilitative needs, and support person‑centered planning. Responsibilities Conduct needs assessments and develop care plans for aging or disabled individuals. Deliver ongoing case management using assessment tools and care plans. Ensure principles of Person‑Centered Planning are followed in service delivery. Develop support plans that include formal and informal services, identifying choices and options. Coordinate and consult with the recipient’s care team. Assess the need for alternative decision makers and collaborate with them. Monitor support plans and health/safety status through face‑to‑face contacts at least every six months or more frequently as needed. Re‑evaluate support plans and modify services as required. Maintain accurate and timely documentation to maximize reimbursement for case management services. Perform intake duties, including assessment activities and MnCHOICES assessment certification as applicable. Conduct face‑to‑face reassessment screenings and consult with other team members per state statute. Co‑ordinate with transitional service providers for eligible individuals. Provide consultation to nursing home residents, collaboration with nursing homes and transitional services. Provide resource information and referrals to community members seeking long‑term care services. Promote continuous quality improvement through ongoing process planning. Ensure case notes, care plans, and service agreements are complete and timely. Maintain knowledge of consumer‑directed services and data practice rules. Participate in training and certification requirements, staying current with federal, state, and department regulations. Collaborate with other county departments and community committees as appropriate. Perform additional duties as assigned. Minimum Required Qualifications Eligibility under the Minnesota Merit System as a Social Worker, Public Health Nurse, or Registered Nurse. Desired Qualifications Bachelor’s degree in social work, psychology, sociology, or closely related field; or any field with one year of social work experience in a public or private agency. One year of satisfactory paid performance in a Home and Community Based services setting. Public Health Nurse with at least one year of home and community‑based experience. Registered Nurse with at least two years of home and community‑based experience. Knowledge, Skills, and Abilities Knowledge of Medicaid and Medicare rules and guidelines. Ability to objectively evaluate individual needs and advocate non‑judgmentally. Basic math skills and organizational/time‑management abilities. Ability to maintain positive relationships with county offices, Health & Human Services, other departments, and state agencies. Knowledge of HIPAA and Data Practice Privacy Rules. Tools and Equipment Used Computer, copier, phone, fax, automobile, and other miscellaneous equipment related to the position. Physical Demands Requires sitting, walking, using hands and arms, and occasional lifting of up to 50 pounds. Must be able to drive and operate a vehicle. Work Environment Combination of office and client homes. Exposure to adverse weather, noise levels ranging from quiet to loud, and potential contact with contagious diseases or individuals/cats/dogs.
