$53 - $56 hourly

Responsibilities:

• Coordinate and manage a caseload of hospice patients, ensuring personalized care plans are developed and executed.
• Conduct regular home visits to assess patient needs, providing compassionate support to both patients and their families.
• Collaborate with interdisciplinary teams to ensure comprehensive care that addresses physical, emotional, and spiritual needs.
• Educate patients and families on hospice care processes, empowering them with knowledge and confidence.
• Maintain accurate and timely documentation of patient care activities, ensuring compliance with regulatory standards.
• Serve as a liaison between patients, families, and healthcare providers, facilitating effective communication and care coordination.
• Participate in team meetings to discuss patient progress and contribute to care plan adjustments as needed.

Qualifications:

• Current Registered Nurse (RN) license in the state of New Jersey.
• Experience in hospice or palliative care, with a deep understanding of end-of-life processes.
• Ability to develop and manage personalized care plans tailored to individual patient needs.
• Proven track record of effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage a diverse caseload efficiently.
• Ability to conduct thorough home visits, assessing and addressing patient needs with compassion.
• Proficiency in maintaining accurate and timely documentation, ensuring compliance with healthcare standards and regulations.
