SOAR Case Manager Home & Community-Based Support

Elizabeth Layton Center

Elizabeth Layton Center seeks a SOAR Case Manager to provide case management services for consumers with severe and persistent mental illness. This role includes assisting clients with Social Security Benefits and collaborating with other mental health staff. The ideal candidate will have a Bachelor's degree and familiarity with mental health practices, along with strong communication skills and a valid driver's license.
