We are seeking a compassionate, dedicated Master of Social Work (MSW) graduate

to join our team. In this role, you will provide essential psychosocial assessments,

emotional support, grief counseling, and community resource planning to patients and

families navigating end-of-life care.

We welcome both seasoned hospice veterans and passionate new graduates!

We are open to apprentices or recent graduates, provided you hold a license that meets Medicare Hospice Conditions of Participation to practice independently in the State of Texas.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and families within the Greater Houston Area.
• Develop, implement, and update individualized plans of care in collaboration with our interdisciplinary hospice team.
• Provide emotional support, crisis intervention, and grief/bereavement counseling to patients and their loved ones.
• Connect families with community resources, financial assistance, and support systems.
• Maintain accurate, compliant, and timely electronic clinical documentation in accordance with Medicare guidelines.

Qualifications & Requirements:

• Education: Master of Social Work (MSW) degree from an accredited university.
• Licensure: Active Social Work license in the State of Texas (LMSW or LCSW) compliant with Medicare hospice guidelines for independent practice.
• Experience: Hospice or home health experience is a plus, but we are open to training the right candidate who aligns with our core values.
• Travel: Must have a reliable vehicle, valid Texas driver's license, and auto insurance.
• Traits: High level of personal integrity, excellent communication skills, and a heartfelt desire to provide premium end-of-life care.

Compensation & Benefits:

• Competitive Salary: $75,000 – $90,000 annually (based on experience).
