About This Role The RN Care Coordinator in Adolescent Medicine at Cleveland Clinic's Main Campus is responsible for managing high‑risk patients’ care, coordinating services, and providing support to patients and their families. Highlights Pay: $63,250.00 - $96,467.50 annually Location: Cleveland, OH Facility: Cleveland Clinic Department/Unit: Main Campus Schedule: Full‑time, Day shift What You’ll Do Work collaboratively with a multidisciplinary care team for high‑risk patients. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments, including medical and social needs. Inform and work with patients and families regarding care coordination. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources. Define and ensure compliance with disease‑specific care paths. Coach patients and families on self‑management support. Requirements Education Graduate from an accredited school of Professional Nursing. Bachelor’s of Science in Nursing (BSN) preferred. Experience 3-5 years of nursing experience. Licenses & Certifications Current RN License in Ohio. BLS certification through AHA or American Red Cross required. Specialty certification preferred.
