RN Care Coordinator - Pediatric Endocrinology

About This Role As an RN Care Coordinator in Pediatric Endocrinology at Cleveland Clinic, you will work collaboratively with a multidisciplinary care team to enhance patient care and outcomes. This role is ideal for dedicated nurses looking to make a significant impact on patients' lives while furthering their own nursing skills. Highlights Pay: $63,250.00 - $96,467.50/year Location: Cleveland, OH Schedule: Full-time, Day shift (8:00am—4:30pm or 8:30am—5:00pm) What You'll Do Work collaboratively with a multidisciplinary care team for high-risk patients. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments addressing various patient needs. Educate and coach patients and families on care coordination. Serve as a liaison and advocate for patients and families. Manage transitions of care across settings, ensuring optimal communication. Identify barriers and connect patients to community resources.
