About This Role As an RN Care Coordinator in Pediatric Endocrinology at Cleveland Clinic, you will work collaboratively with a multidisciplinary care team to enhance patient care and outcomes. Highlights Pay: $63,250.00 - $96,467.50/year Location: Cleveland, OH Schedule: Full-time, Day shift (8:00am—4:30pm or 8:30am—5:00pm) What You'll Do Work collaboratively with a multidisciplinary care team for high-risk patients. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments addressing various patient needs. Educate and coach patients and families on care coordination. Serve as a liaison and advocate for patients and families. Manage transitions of care across settings, ensuring optimal communication. Identify barriers and connect patients to community resources. Requirements Education: Graduate from an accredited school of Professional Nursing Experience: 3-5 years of nursing experience Licenses & Certifications: Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through AHA or American Red Cross Benefits Comprehensive healthcare, dental and vision benefits Retirement savings account contributionsAdditional benefits not specified in detail #J-18808-Ljbffr IntelliResume
