Our team is passionate about improving patient outcomes, and providing compassionate care in every interaction.---**Position Summary:**We are seeking a skilled and compassionate **Paramedic** to join our growing team at **Hospital to Home LLC**. In this role, you will provide advanced medical care in the field, both in emergency and non-emergency settings. You’ll be a key player in our mission to bridge the gap between hospital care and home health, ensuring safe, efficient, and clinically appropriate medical transport and support.---**Key Responsibilities:*** Respond to calls for medical assistance, interfacility transfers, and in-home assessments.* Provide advanced life support (ALS) care including medication administration, airway management, cardiac monitoring, and trauma stabilization.* Collaborate with RNs, case managers, and other healthcare professionals to support seamless patient transitions from hospital to home.* Monitor and document patient condition during transport, communicating any changes to medical personnel and receiving facilities.* Operate and maintain medical equipment and the ambulance in a state of readiness.* Ensure timely, accurate, and complete documentation of all patient care provided.* Educate patients and families on medications, follow-up care, and health management when applicable.* Uphold patient confidentiality and professional standards in all interactions.---**Working Conditions:*** Variable shifts including nights, weekends, and holidays.* Work may involve physically demanding tasks and exposure to varying environmental conditions.* High-paced environment that requires flexibility and critical thinking.---**Why Join Hospital to Home LLC?*** Opportunity to be part of a progressive, patient-focused healthcare model* Competitive pay and benefits* Supportive team culture and ongoing training* Meaningful work that truly impacts lives and communities---**Hospital to Home LLC is an equal opportunity employer.** We welcome applicants from all backgrounds and are committed to a diverse and inclusive workplace.## Qualifications**Qualifications:*** High School Diploma or GED required; Associate’s Degree in EMS or related field preferred.* Current **State Paramedic License** (and NREMT if required by state).* Current certifications: **BLS**, **ACLS**, **PALS**; **ITLS/PHTLS** preferred.* Valid driver’s license for more than 3 years and acceptable driving record.* EVOC certification or equivalent preferred.* Strong interpersonal and clinical assessment skills.* Prior experience in EMS, mobile healthcare, or community paramedicine a plus.* Compassionate, detail-oriented, and committed to patient-centered care.* Must maintain certification through OEMS as Paramedic* Must be 21 years of age or older* Must be able to lift 125 lbs. individually and 250 lbs. assisted.
