PM RN Admissions Nurse - Care Transitions & Assessments

Arden Park Post Acute

Arden Park Post Acute is seeking an experienced RN Admissions Nurse to coordinate new resident admissions, perform clinical assessments, and manage care transitions on the PM shift. You will work with physicians, discharge planners, therapy teams, nursing staff, residents, and families to ensure a smooth, compliant admission process. The ideal candidate holds an active California RN license, has at least two years of RN experience, and familiarity with PCC or other EHRs. #J-18808-Ljbffr Arden Park Post Acute
