As part of our compassionate, high‑performing care team, you’ll help individuals improve communication, swallowing safety, and overall quality of life through personalized, evidence‑based therapy.This flexible role is perfect for clinicians who value autonomy, meaningful patient relationships, and a supportive organization behind them.If you are looking for an opportunity to grow your career and make an impact in a supportive, team-focused environment, we would love to hear from you.How will you make an impact & RequirementsWhat You'll DoPatient Assessment: Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.Treatment Planning: Develop individualized treatment plans that address specific patient goals, including speech sound production, language development, fluency, voice disorders, and swallowing difficulties.Therapeutic Interventions: Provide direct therapy services, such as articulation therapy, language therapy, fluency therapy, voice therapy, and swallowing therapy, using evidence-based practices.Patient Education: Educate patients and their families on strategies to improve communication skills, swallowing safety, and overall quality of life.Collaboration: Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to develop comprehensive care plans and ensure optimal patient outcomes.Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.What We're Looking ForLicensure: Current and valid Speech-Language Pathologist (SLP) license in the state of Florida.Experience: Minimum of 1 year of experience in hospital, rehab, or skilled nursing facility is required.Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.Transportation: Reliable transportation to visit patients in their homes.About Millennium Physician GroupFormed in 2008 and headquartered in Fort Myers, Florida, with offices in Florida, North Carolina, Georgia, and Texas, Millennium Healthcare is the largest independent physician group in the state of Florida and one of the largest in the United States.At Millennium Physician Group, our employees are the foundation of our success.
