Weare seeking a skilled and compassionate Physical Therapist to joina Skilled Nursing Facility in Woodland,California. This role involves working with ageriatric and post-acute population, helping patients recover fromsurgery, illness, or injury while improving mobility and overallfunction. You will be responsible for evaluating patients,developing individualized treatment plans, and guiding them towardgreater independence and quality of life. If you are dedicated topatient-centered care and rehabilitation, this is a strongopportunity for both professional growth and meaningfulimpact.

Job Responsibilities

Evaluate, diagnose,and treat patients with physical impairments in a SkilledNursing Facility setting
Develop and implementindividualized treatment plans based on patient needs andclinical findings
Supervise and collaborate with PhysicalTherapist Assistants to ensure effective care delivery
Monitor patient progress and modify treatment plans asneeded
Educate patients and caregivers on mobility,safety, and home exercise programs
Maintain accurate andtimely documentation in compliance with facility and regulatorystandards

Requirements

Active state license as a Physical Therapist in California
Current BLS/CPR certification
Minimum of 1 year of recent PTexperience preferred, ideally in a SNF or geriatric setting
Strong clinical reasoning and leadership skills
Ability towork collaboratively within an interdisciplinary team
