Schedule Information Shift: 18:45 - 07:15 Unit Specific Information NICU (Neonatal Intensive Care Unit) specialty requiring ventilator management expertise Additional Details Must have proof of 2 MMR vaccinations or IGG titer prior to start. Hepatitis B Surface Antibody (titer) or declination accepted; vaccination series recommended. QuantiFERON or T-Spot TB test within 12 months prior to assignment start date; for past positive, CXR and updated TB questionnaire required annually. Proof of 2 Varicella vaccinations or IGG titer required prior to start. Physical/Health Statement attestation required, signed by MD, DO, NP, or PA. Drug screen must be completed within 60 days prior to start date. Background check within 30 days prior to assignment start, including criminal and sanction searches. Employment verification for previous 7 years required. Two references from Manager/Supervisor and above required. I-9 E-Verify attestation required. Color photo for badge required, taken within past 6 months. Skills checklist current within 12 months prior to assignment start date.
