Overview
Licensed Practical Nurse (LPN) – Long Term Care travel assignment in Clifton Springs, NY. Start date 08/25/2026 for 13 weeks. Weekly pay range $1,475–$1,550. Shift: 5x8 evenings (1500–2300). Setting: Nursing Facility / Nursing Home. Travel - Temporary Assignment. Active New York single-state LPN license required to submit; EPIC charting experience is a firm requirement.

Responsibilities
- Administer prescribed medications and treatments per physician orders; document administration in charting system
- Provide skilled nursing care and support activities of daily living; monitor vital signs and assess changes in patient condition
- Obtain 12-lead ECG when ordered; respond to cardiac events and update care plans as directed
- Maintain accurate EPIC charting; communicate with the RN, charge nurse, and interdisciplinary team
- Observe for and report changes in patient status; implement de-escalation techniques as needed; support behavior management
- Collaborate with PT, Radiology, Social Services, and Pharmacy to coordinate patient care; participate in care planning and rounding
- Float within the defined skillset as needed and maintain patient safety and dignity

Requirements
- Active New York LPN license (single-state) in hand to submit
- BLS certification
- EPIC charting experience (firm requirement)
- Minimum 1 year nursing experience; Nursing Facility / Nursing Home experience required
- LTAC experience listed as required; previous charge experience required
- Travel experience required; Accepts first-time travelers
- Ability to work in a Nursing Facility / Nursing Home setting; Floating within scope as needed
- RTO disclosure required at submission; candidates with >10 days RTO will not be submitted
