Wound Care RN - Full Time Day Shift

EDUCATION:  

  • Graduate of an accredited school of nursing with current New York State licensure as a Registered Professional nurse.  

  • Prefer Bachelor of Science in Nursing. 

SKILLS: 

  • Minimum of two (2) years’ experience in medical/surgical, rehabilitation nursing. 

  • Certified Wound, Ostomy and Continence Nurse (CWOCN) preferred

  • Certified Wound Care Nurse (CWCN) required 

 

JOB SUMMARY:

Coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for residents throughout the facility.  Provides resident, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies.  Collaborates with Physicians, Surgeons and clinical staff including set scheduled wound care rounds.  Must be well motivated, organized and able to prioritize multiple tasks.  Must be able to interact effectively with a wide variety of people.  Excellent communication skills.  Able to serve as an example to other employees. 

Specific Requirements

  • Working knowledge of state and federal regulations.
  • Must be able to read, speak, write and understand the English Language.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with personnel, residents and family members.
  • Must possess the willingness to work harmoniously with professional and non-professional personnel.
  • Must possess patient, tact and a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents, if necessary.
  • Demonstrates commitment to continuing professional development.
  • Ability to recognize and later describe persons associated with nursing homes safety and security. 

Working Conditions

  • Works in office areas as well as throughout the nursing service area (i.e. medication, resident’s rooms, etc.)
  • Sits, stands, bends, lifts and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies, under all conditions/circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel and visitors.
  • Communicates with medical staff, nursing personnel.
  • Works beyond normal working hours and on weekends, holidays when necessary.
  • Is subject to falls, burns from equipment, odors, etc throughout the workday.
  • Is subject to exposure to infectious waste, diseases, conditions, including AIDS and Hepatitis B Viruses.
  • Physical and Sensory Requirements (With or without the aide of mechanical devices.)
  • Must be able to cope with the mental stress and emotional stress of the position.
  • Must be able to hear and see or use prosthetics that will enable these senses to function.
  • Must be in general good health.
  • Must be able to lift, pull, push and move a minimum of fifty pounds.
  • Must be able to assist in the evacuation of residents.

Employee will receive in service education for all mandatory topics, upon hire, and at least, annually thereafter. The facility Administrator and/or designee will be responsible for conducting observations, evaluations and competencies of the employee to ensure compliance and competency. Employee is expected to follow all facility policies and procedures referenced upon hire and as needed including timeliness, attendance, confidentiality, and professionalism. Attire is expected to be neat, hygienic, and professional. 

DUTIES AND REQUIREMENTS – 

 

  1. Assess the unique needs of each resident to determine appropriate practices to prevent skin breakdown and promote skin wellness. 

  2. Ability to perform wound assessment, reassessment and management for all residents as per policy. Track treatments and resident’s progress.    

  3. Knowledge of the anatomy and physiology and dermal systems. 

  4. Utilizes principles of current wound management theory when providing care to residents with acute and chronic wounds. 

  5. Demonstrates thorough knowledge of staging system of wounds. 

  6. Develops skin care programs to prevent skin breakdown of residents. 

  7. Demonstrates knowledge and skill to develop and implement policies and procedures to manage residents with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care and incontinence. 

  8. Ability to revise plan of care as indicated by the resident’s response to treatment and evaluate overall plan for effectiveness. 

  9. Ability to adequately assess and reassess pain.  Utilizes appropriate pain management techniques.  Educates the resident and family regarding pain management. 

  10. Demonstrates ability to perform treatments as scheduled and as needed. 

  11. Ensure that all plans for preventative skin care are in EMR and Nursing Instruction. 

  12. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate. 

  13. Knowledgeable of medications and their correct administration based on age of the resident and clinical condition. 

  14. Is a resource person to physicians, nursing and other hospital staff.  Provides in-services on wound prevention, wound care, ostomy care and incontinence for the nursing staff. 

  15. Communicates and coordinates appropriately and clearly to nurse managers, nursing staff and physicians. 

  16. Consults other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the resident’s needs. 

  17. Documentation meets current standards and practices. 

  18. Attends seminars, conferences, continuing education classes in the areas of wound prevention, wound management and ostomies.  Participates in professional societies.  Stays current with changes, new advances/treatment in wound care. 

  19. Serves as a liaison between physicians, residents and the Interdisciplinary Team. 

  20. Knowledgeable of the clinical products.    Meets with sales representatives, evaluates products; makes recommendations for products/equipment.  

  21. Operates all equipment correctly and safely. 

  22. Coordinates and supervises resident care as necessary. 

  23. Demonstrates an ability to be flexible, organized and function under stressful situations. 

  24. Inputs charges and enters documentation on all wound care residents into computer system in a timely manner to ensure compliance. 

  25. Follows all Infection Control Procedures and protective measures, including appropriate PPE.  This includes all procedures or job-related tasks that involve inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or potential spills or splashes of them.

  26. Job duties will be assigned as needed which includes emergencies.
