Qualifications & Requirements

• Graduate of a physical therapy program accredited by the APTA or equivalent
• Current Physical Therapist license in Florida, or willingness to obtain (Florida PT license)
• Minimum 1 year of PT experience preferred; new graduates considered (entry-level home health PT jobs)
• Prior home health experience preferred; experience with in-home evaluations and skilled nursing documentation a plus
• Proficiency with NetSmart EMR and completion of OASIS documentation
• Must meet CEU and HIV licensure requirements
• Valid Florida driver's license, clean driving record, proof of personal auto insurance (use of own vehicle required for patient visits)
• Ability to meet productivity expectations (31 points per week)

Key Responsibilities

• Perform comprehensive physical therapy assessments addressing medical, psychosocial, and environmental needs in the home setting
• Develop, implement, and update individualized Plans of Care (POC) and measurable rehabilitation goals in collaboration with physicians, patients, caregivers, and the Clinical Manager
• Complete accurate OASIS assessments and timely EMR documentation using NetSmart
• Obtain and follow physician orders; communicate changes in patient condition and report progress
• Provide patient and caregiver education on safety, home exercise programs, fall prevention, and functional mobility
• Participate in weekly case conferences, continuing education, and in-service training to support clinical excellence
• Provide leadership to ancillary personnel and assist with orientation and training of new staff
• Maintain client confidentiality and ensure HIPAA compliance in all communications and documentation
• Represent the agency at community health/job fairs and support agency outreach efforts to grow referral streams
