Public/Community Health Nurse Practitioner

We are seeking a compassionate, mission-driven Nurse Practitioner to provide low-barrier, trauma-informed medical care to underserved and vulnerable populations throughout the Chicago area. This role focuses on delivering care through a Harm Reduction framework in mobile outreach, street medicine, shelters, youth programs, and community-based settings while helping patients connect to ongoing healthcare and support services.
