Experienced Case Managers Needed for Temple, Tx

Job Description Lifespan Home Health is seeking a " Case Manager " to help support our growing Temple, TX office. We are an established and stable home care agency that provides in-home, non-medical care to the elderly and children and adults with disabilities so they can remain safe and comfortable in their home, with their family, and in the community. The Case Manager manages provider services programs (i.e. STAR+PLUS, Primary Home Care, Family Care, STAR Kids) and provides DIRECT personal care as needed to patients and families in accordance with Agency policies and scope of practice. Essential Tasks Coordinates and facilitates Starts of Care and Supervisory Visits with clients. Communicates with State program and MCO Case Workers and Service Coordinators regarding service breaks and/or client issues in a timely manner. Supervises personal care attendants. Responsible for assisting with billing of services, scheduling services, and office staff interactions. Frequent interaction with Human Resources Department to ensure staff files are up to date. Knowledge of DADS rules and regulations is helpful. If you have experience with STAR+PLUS, MDCP, PHC, or STAR Kids programs, we want to talk to you! Requirements High school diploma/GED required PAS program management experience preferred. Minimum of two (2) years’ experience as a supervisor. Willingness to work with patients, DADS Caseworkers and MCO Service Coordinator. Great verbal and written communication skills and ability to communicate in Spanish preferred.
