Case Manager Registered Nurse - San Antonio, TX

Remote RN Case Manager (Costco Team) Join our Costco team as a Remote RN Case Manager, where you’ll play a key role in helping members achieve better health outcomes through compassionate, coordinated care. Responsibilities Conduct telephonic (and occasional in‑person) assessments to understand member needs. Develop and manage personalized care plans to improve health and wellness. Coordinate care across programs, providers, and resources. Identify risks and remove barriers to care using clinical judgment. Engage members through meaningful conversations to support their health goals. Collaborate with a multidisciplinary team to optimize outcomes. Qualifications Active, unrestricted multistate RN license in state of residence. 3+ years of clinical nursing experience. Strong computer skills (Microsoft Office and web‑based systems). Ability to work the required schedule. Associate’s Degree in Nursing (preferred). Case‑management experience in an integrated model (preferred). Bilingual (Spanish/English) (preferred). BSN (preferred). Located within 45 minutes of San Antonio, TX for occasional in‑person visits—remote otherwise. Schedule & Location Remote position. Full time, Monday–Friday 8:00 AM–5:00 PM CST with occasional rotation to 10:00 AM–7:00 PM CST. No weekends or holidays. Compensation Base salary range: $60,522.00 – $129,615.00. This range represents the base hourly rate or base annual full‑time salary. Total compensation may include bonus, commission or short‑term incentive programs.
