What You'll Do:

The ACT Nurse Case Manager is an integral member of the Assertive Community Treatment team and provides nursing, medication, physical health, care coordination, rehabilitation, and support services in community-based settings. The position supports individuals in advancing their self-identified goals related to recovery and wellness. Nursing services are delivered in collaboration with the psychiatric care provider, ACT team, primary care providers, pharmacies, hospitals, community partners, and each individual’s identified supports, when authorized. Responsibilities include:

• Complete nursing assessments and ongoing health monitoring 
• Collect and document relevant physical, behavioral, medication, social, cultural, and functional health information using established guidelines, tools, and workflows.
• Administer medications and implement medical orders as permitted by licensure and consistent with prescriber orders, the established plan of care, and organizational procedures.
• Monitor, document, and report medication effectiveness, adherence, side effects, adverse reactions, complications, and individual-reported concerns to the psychiatric care provider and ACT team.
• Participate in assessment of current and historical medical concerns, engagement with primary care, preventive care needs, and barriers to accessing health services.
• Assist individuals in establishing and maintaining connection with a primary care provider, dentist, specialists, preventive services, and community health resources.
• Assist individuals with movement across levels of care and connection to recovery education, self-help, substance use treatment, residential, vocational, and community resources based on identified goals and preferences.
• Complete service notes, nursing documentation, medication records, care coordination records, incident reports, and other required documentation accurately and within established timeframes.

Disclaimer:

This role requires flexibility to transport clients/patients to organization-sponsored services or resources that address their basic and clinical needs. Drivers transporting clients/patients or driving company vehicles are to be age 21 or greater. For those using their personal vehicles to transport clients/patients, a certificate of insurance is required demonstrating at least the minimum required NC auto liability insurance.

What We're Looking For:

• Graduation from an accredited school of nursing required.
• Current, unrestricted North Carolina license as a Registered Nurse or Licensed Practical Nurse, or a multistate/compact license authorizing nursing practice in North Carolina, required.
• One year of psychiatric, behavioral health, community health, or related nursing experience required. 
• Knowledge of nursing theory and practice, psychotropic and other commonly used medications, medication administration and monitoring, physical health conditions, care coordination, recovery-oriented practice, and community resources.
• Ability to use the electronic health record, medication administration records, Microsoft Office applications, and other systems required for the role.
• Ability to work a flexible schedule, participate in assigned on-call or after-hours coverage, provide services in community settings, and respond to changing service needs.
• Valid driver’s license and driving record acceptable under BRH policy required.
