Christian Hospital and Northwest HealthCare is seeking a compassionate RN Case Manager to coordinate patient care, facilitate safe discharges, reduce avoidable admissions, and connect patients with post-acute resources. The role is full-time, days, with rotate weekend and holiday coverage. Ideal candidates have a Nursing diploma/associate degree (BSN preferred) and 2–5 years in case management, with EPIC experience preferred.
