JOB SUMMARY:

We are seeking a compassionate and dependable Caregiver to join our Assisted Living and Memory Care community. In this role, you will provide hands-on assistance with daily living activities, ensure resident safety, and foster a positive, engaging atmospheres for all residents, including those receiving specialized support in our Memory Care community.

RESPONSIBILITIES:

• Provide assistance with activities of daily living (ADLs)
• Provide companionship, emotional support, and meaningful engagement to residents
• Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports
• A Caregiver will maintain resident confidentiality
• Encourage clients to participate in activities that promote physical, mental, and emotional well-being
• Support clients in maintaining independence in their daily routines and decision-making
• Ensure a safe, clean environment by identifying and addressing potential hazards in community settings
• Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols
• Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care
• Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes
• Present for staff meetings and training
• Monitor and document changes in residents' physical, emotional, or cognitive conditions and promptly report concerns to nursing staff or supervisors
• Assist residents during mealtimes, including serving meals, encouraging proper nutrition, and providing feeding assistance when needed
• Follow individualized care plans and comply with state regulations, company policies, and infection control protocols
• Other duties as assigned

RESPONSIBILITIES:

• Must be 18 years of age
• High school diploma or equivalent required
• Previous experience in caregiving, home care, or community-based care is preferred
• Ability to work independently and as part of a team
• Patience, empathy, and a genuine desire to help others
• Good organizational and time management skills
• Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility
• Must be able to stand, walk, and reach for extended periods
• Ability to pass a background check

WHO WE ARE: One of Sapphire's newest communities, McKenzie Manor, formerly SouthTowne was acquired in November 2022. This community is 100% memory care and is comprised of 47 total units. Recently we have repainted the entire interior and rolled out a brand new furniture package. The grounds have had many improvements as well including renovations to the entry way.
