The Postgraduate Center for Mental Health seeks a Licensed Mental Health Professional to join a NY OMH-funded Adult Home based care transition team. The role focuses on transition planning, discharge readiness, developing daily living skills, and coping strategies through group and individual work at the Adult Home.
