Nurse Practitioner

Advanced Practice Clinician- Nurse Practitioner/Physician AssistantThis Advanced Practice Clinician will support our client's Senior Community Care (SCC) program which is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. In this role you will have the ability to achieve work life balance. Flexible scheduling offered where providers can flex their with no on-call, no weekends, and no holidays required.Primary Responsibilities:Conduct comprehensive assessmentsEffectively manage acute and chronic medical and behavioral conditionsEnsure accurate and complete documentation, including ICD 10 conditionsCommunicate and collaborate with the interdisciplinary care teamConduct advanced illness and advanced care planning conversationsProvide patients and caregivers with counseling and educationRequired Qualifications:Active and unrestricted license Physician Assistant or Nurse Practitioner license in New York stateCurrent, active DEA licensure/prescriptive authorityAccess to reliable transportationAbility to gain a collaborative practice agreement, if applicable in your stateFull COVID-19 vaccination is an essential requirement of this role.Preferred Qualifications:1+ year of clinical experience in practice (long-term care setting preferred)Understanding of Geriatrics and Chronic IllnessUnderstanding of Advanced Illness and end of life discussionsProficient computer skills including the ability to document medical information with written and electronic medical recordsMonday - Friday 8 AM to 5 PM Pay Range: $60-70/hour
