Registered Nurse Home Health

Shift: 8hrs

Contract Weeks: 13, Guaranteed Hours: 40

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• CHPN certification (Preferred)

Roles & Responsibilities:

• Assess patients in various situations to identify their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care (POC) for quality and continuity of care.
• Educate patients and families on care, medications, and self-care practices.
• Supervise and provide clinical direction to support staff to ensure continuity of services.
• Manage pain assessments and implement appropriate pain management techniques.
• Communicate effectively with patients, families, and interdisciplinary team members.
