LPN Job Requirements

Active Licensed Practical Nurse (LPN) license in good standing
Current BLS certification
LPN experience preferred
Home health experience strongly preferred
HCHB experience is a plus
Associate degree
Ability to work independently and manage patient visits efficiently
Professional appearance with solid-color scrubs

Responsibilities

Provide compassionate patient care in the home health setting
Support patients with routine nursing visits and ongoing care needs
Document care accurately and maintain timely clinical records
Collaborate with the care team to support positive patient outcomes
Work within the designated coverage area and maintain efficient daily scheduling
Deliver safe, high-quality nursing care with professionalism and attention to detail
