• Promotes Elara Caring's philosophy and mission to ensure quality care.
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on assessments and collaboration.
• Directs support staff to achieve patient functional goals.
• Documents evaluations, treatment goals, and plans, maintaining prescriptions and signatures.
• Provides leadership and acts as a resource to promote physical therapy in the community.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Able and willing to travel within branch/office coverage area (about 50%).
• Must have a dependable vehicle, valid drivers license, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.
