Physical Therapist Home HealthPinellas and East and West Pasco Counties Seeking a Per Diem Physical Therapists (PT) to join our team in a Home Health setting! This is an exciting opportunity to join an evolving therapy team and deliver exceptional care for patients and families receiving Home Health Services.Conduct patient assessments and evaluations to determine their physical condition, limitations, medical histories, and personal goals.Evaluate patient's strength, range of motion, neurological status, balance, and ambulation or wheelchair mobility.Provide direct patient care under the authorization of a physician.Create and update treatment plans, testing, measuring the patient's strength and flexibility.Confer with physicians, patients, families, nurses, therapists, social workers, and members of the patient care team.Develop a treatment plan for patients with specific goals and tasks to assist in meeting functional goals.Evaluate patient's home and identify potential improvements based on their needs.Complete all clinical notes using the company's EMR system.Contribute to meetings and case conferences to ensure coordinated and comprehensive care plans for patients.Oversee physical therapy assistants and physical therapy aides.Current Florida Physical Therapy License with minimum two years of experience.We are an equal opportunity employer and prohibit discrimination/harassment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
