Occupational Therapist
Job Summary:
We are seeking a skilled and compassionate Occupational Therapist to join our dedicated team at The Grand Rehabilitation and Nursing at Rome. As a key member of our healthcare team, you will play a vital role in evaluating and developing treatment plans for our residents, promoting their functional independence and overall well-being.
Responsibilities:
Evaluate and develop treatment plans for residents based on medical diagnosis
Implement treatment plans to improve residents' goals toward their functional independence
Assess and document the progress of residents in detailed recertifications, progress reports, and daily notes
Maintain resident confidentiality by monitoring confidential information processing
Maintain documentation of resident care services by auditing resident and department records
Clinically supervise occupational therapy assistants and participate in care plan meetings and documentation
Plan, develop, and organize OT-related reports, data tracking, and other projects as directed by the Rehab Director
Assist in departmental projects as needed
Requirements:
Graduate of an accredited Occupational Therapy Program
Current New York State OT License
Experience is a plus
