Conduct patient evaluations in both inpatient and outpatient settings and create customized treatment plans

Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and functionality

Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes

Guide patients and their families regarding therapeutic objectives, discharge plans, and at-home exercises

Maintain thorough documentation of evaluations, treatments, and progress in accordance with facility policies and regulatory standards

Engage in treatment team meetings and interdisciplinary conferences to ensure seamless care coordination

Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution

State licensure as a Physical Therapist

CPR certification is preferred

Any specialized certifications (such as in neurology or geriatrics) are advantageous but not mandatory

Previous experience is highly preferred

Competence in treating patients with intricate medical and mobility challenges

Exceptional communication and teamwork abilities

Familiarity with digital charting and clinical documentation systems is beneficial
