Registered Nurse – Case Management / Discharge Planning
Position Overview:
We are seeking an experienced Registered Nurse (RN) to perform case management and coordinate the discharge planning process for age-appropriate inpatients with chronic diseases and special healthcare needs.

The RN will work across the Multi-Service Unit, Intensive Care Unit (ICU), OB Units, and Adolescent Care Unit, coordinating care and facilitating appropriate transitions between healthcare facilities and home.

Key Responsibilities:
Perform case management and coordinate the inpatient discharge planning process.

Manage patients with chronic diseases and special healthcare needs.

Maintain tracking systems for patients referred for admission or transfer to other healthcare facilities.

Perform chart reviews for requested services and determine appropriate arrangements for return to a facility or discharge home.

Coordinate with healthcare providers and other members of the care team to facilitate safe and timely discharge.

Attend staff meetings as scheduled.

Required Experience:
2+ years of experience in discharge planning REQUIRED.

Med-Surg experience REQUIRED.

RN experience in a clinical, medical/surgical, acute care, and/or critical care setting REQUIRED.

License & Certifications:
Valid and unrestricted Registered Nurse (RN) license in any U.S. state or territory.

Current American Heart Association (AHA) BLS certification required and must be maintained throughout the assignment.
