Exciting Opportunity for a Case Management RN!

Job Description:


- Join our dynamic team as a Case Management RN, where you'll play a pivotal role in making a difference in the lives of patients!
- Work Monday to Friday, from 8:00 a.m. to 5:00 p.m., with a one-hour lunch - enjoy your evenings and weekends!
- Perform case management and coordinate the discharge planning care process for age-appropriate in-patients with chronic diseases and special health care needs.
- Engage with Multi-Service Units, Intensive Care Units, OB Units, and Adolescent Care Units.
- Maintain a comprehensive tracking system for referred patients and ensure smooth transitions back home or to other healthcare facilities.
- Conduct thorough chart reviews for requested services to facilitate successful discharges.

Requirements:


- Minimum of two (2) years of experience in discharge planning and Med-Surg settings.
- Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
- Strong clinical background with RN experience in acute and/or critical care settings.
- Valid American Heart Association (AHA) certification in Basic Life Support (BLS); must maintain certification.

Additional Details:

- All Travelers must attend New Employee Orientation, scheduled every other Monday.
- Ensure you are cleared seven (7) days prior to your start date to avoid any delays.
