Youth Resiliency Program Case Manager

The Youth Resiliency Program (YRP) provides evidence-based, trauma-informed services to moderate- to high-risk youth. The program delivers individual therapy, youth and family groups, case management, and wraparound supports to prevent deeper justice involvement and strengthen families, schools, and communities.

The Youth Resiliency Program Case Manager provides individualized case management, care coordination, advocacy, and practical support to youth and families. This position helps participants address behavioral health, school, family, and community needs; access appropriate services; and make progress toward long-term stability and well-being.

Role and Responsibilities

The primary duties include but are not limited to the following:

Referral, Engagement and Enrollment

• Build and maintain referral relationships with Fulton County schools, courts, behavioral health providers, and community partners.
• Receive and review referrals, confirm program eligibility, and promptly engage youth and parents or guardians.
• Coordinate intake requirements, including informed consent and referral for a behavioral health assessment before services begin.
• Explain program services, expectations, participant rights, confidentiality, and available supports to youth and families.
• Identify immediate behavioral health, educational, family, transportation, housing, and safety needs and coordinate appropriate next steps.

Case Management and Wraparound Support

• Develop individualized, strengths-based service plans with youth and families and regularly review progress toward behavioral, educational, and personal goals.
• Provide ongoing support, motivation, advocacy, and problem-solving using trauma-informed and culturally responsive approaches.
• Maintain consistent contact with participants and parents or guardians through office, school, home, community, and virtual visits, as appropriate.
• Connect families to mental health and substance use services, school supports, community resources, housing, healthcare, transportation, and other basic-needs assistance.
• Support school engagement through attendance follow-up, homework assistance, communication with school partners, and coordination of GED, college, or career-readiness resources.
• Assist youth with conflict resolution, coping skills, social skills, independent living skills, and positive decision-making.
• Coordinate with therapists, schools, justice-system partners, family members, and other providers to ensure services are aligned and barriers are addressed.
• Respond appropriately to participant concerns and crises, follow safety and incident-reporting procedures, and consult clinical leadership when needs exceed the case management role.
• Provide or coordinate transportation for program-related services and activities in accordance with organizational policy.

Groups and Community Engagement

• Support and, when assigned, co-facilitate youth, family, and peer groups focused on parenting support, social skills, conflict resolution, independent living, and life skills.
• Plan and supervise structured activities, community events, and pro-social experiences that strengthen engagement and resilience.
• Represent YRP at outreach events, partner meetings, and multidisciplinary case conferences.

Documentation, Monitoring and Team Collaboration

• Complete timely, accurate case notes, service plans, assessments, attendance records, referrals, outcome tracking, and required reports.
• Maintain confidential participant records and follow organizational, grant, and legal requirements for documentation and information sharing.
• Monitor participation and progress, follow up on missed services, and use data to identify and address barriers to engagement.
• Participate in regular supervision, case staffing, training, quality-improvement activities, and program meetings.
• Collaborate with program and clinical leadership to support coordinated, ethical, and effective service delivery.
• Perform other duties as assigned.

Requirements

• Bachelor's degree in social work, psychology, criminal justice, human services, or a related field from an accredited college or university, with 1-2 years of related experience.
• Experience providing case management or wraparound services to youth and families, preferably in behavioral health, school, community, diversion, or juvenile justice settings.
• Knowledge of trauma-informed care, adolescent development, family engagement, crisis response, community resources, and confidentiality requirements.
• Strong documentation, organization, time-management, communication, and relationship-building skills.
• Valid Georgia driver's license, acceptable driving record, and ability to pass a criminal background check.

Schedule

This is a full-time position and typically works Monday - Friday, 8:00 a.m. - 5:00 p.m., but requires flexibility based on participant and program needs.

Salary Description $42,000 - $47,000
