Enjoy flexible scheduling while providing care to patients of all ages wherever they call home.Supervisory Responsibilities: NoneDuties/Responsibilities:Makes home visits for patients of all agesUses only approved abbreviationsUnderstands patient safety goals & actions takenUses 2 patient identifiers before giving treatments, demonstrates processUnderstands read back of verbal orders/critical lab values & can state processUnderstands procedure to clarify physician ordersUses standing orders when indicatedAccurately calls report to MD as warranted by condition; reports to other providers as neededReports sentinel events or adverse drug reactions or incidents as warrantedUses final verification before invasive procedures & can verbalize correct process & documents in EHRReassesses patient in response to changes in condition after intervention as needed & documents in EHRProvides treatments (including wound care, venipuncture & IV administration) as ordered in a timely & appropriate manner & documents in EHRCommunicates with & provides education to patients, caregivers, & family members in a timely manner (using respect & dignity when dealing with death & dying) & documents in EHRAssessments/evaluations/resumptions are timely, complete & accurate according to federally mandated time framesHIS/HOPE reporting accurate & complete within time frame established by federal governmentProvides professional assessment (includes nursing, psychosocial & spiritual needs) as well as need for other services & re-evaluates as appropriateCompletes, maintains, & updates treatment care plans in a timely manner, identifying all appropriate problems & establishing realistic goalsConsideration is given to appropriate resources usage including appropriate visit frequencyCommunicates with physicians & refers to other disciplines as appropriateParticipates in case conferencesCommunicates with nursing facilities as neededAttends IDG meetings; provides daily reports to Patient Care CoordinatorDevelops aide care plans & oversees visits according to state & federal regulations & documents accordinglyMaintains compliance with productivity standards according to departmental requirementsTime/visit documentation is accurate & submitted in a timely manner according to departmental requirements & state/federal regulationsDischarges are documented within time frames required by state & federal regulationsPatient outcomes are equal to or better than available benchmarksAssists with chart audits as neededDemonstrates competencies for all populations servedRequired Skills/Abilities:Exceptional communication and listening skillsStrong foundation in nursing skills to include: critical thinking, assessment, care planning, & administering treatmentProficient in time management and adaptabilityMust be able to operate a computer, phone, medical equipment, & automobileEducation and Experience:Minimum Associates degree in nursing, Bachelor's degree preferredA minimum of 1 year related work experience requiredMust be licensed as a Registered Nurse in the state of IndianaPhysical Requirements:Frequent use of hands, walking, sitting, standing, kneeling, reaching, stooping, talkingOccasionally may have to lift over 50 lbsEqual Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
