Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence while enhancing their quality of life.Key ResponsibilitiesProvide physical therapy treatments as directed by the supervising Physical TherapistFollow physician orders and individualized treatment plansDocument daily treatments, weekly progress notes, and resident outcomes accurately and timelyParticipate in patient care conferences and rehabilitation meetings as neededMonitor resident progress and communicate updates to the Physical TherapistAssist with cleaning and maintenance of therapy equipment and treatment areasTrain and assist Restorative Aides in carrying out therapy-related programsCommunicate effectively with residents, families, nursing staff, and interdisciplinary team membersParticipate in staff education and in-service training programsEducate residents, families, and caregivers on maintenance programs and discharge recommendationsReport equipment concerns and assist in maintaining a safe therapy environmentEnsure compliance with all facility policies, state regulations, and payer requirementsQualificationsEducation & ExperienceGraduate of an accredited Physical Therapist Assistant programExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong communication, documentation, and organizational skillsLicenses & CertificationsCurrent South Carolina Physical Therapist Assistant (PTA) license in good standingMust maintain all required continuing education requirementsMust remain in good standing with the applicable State BoardWhat Makes You a Great FitPassion for helping residents regain mobility and independenceStrong clinical and interpersonal skillsPositive attitude and team-oriented mindsetDependable, flexible, and self-motivatedCommitment to delivering exceptional resident-centered careApply TodayJoin River Falls Post Acute and become part of a dedicated rehabilitation team that values your expertise and supports your professional growth.
