Daily Responsibilities: Conduct telephonic prospective, concurrent and retrospective review of active patient care. Coordinates treatment plans and conduct transitional care calls. Prioritizes patient care needs. Developed care plans with time frames and obtainable outcomes. Reevaluate care plans, time frames and outcomes. Qualifications Requirements as Ambulatory Case Manager: Bachelor’s degree in Nursing Active California RN license in good standing 3+ years of Clinical Nursing with 1 of those years being within an Outpatient setting Additional Information Pay Rate: $38/hr. - $41/hr.
