Shift: Day Shift | Mondays - 9-5p & Tuesday, Wednesday, Thursday - 12 noon-8p

Salary Scale: $31.10 - $43.54

Dominion Hospital, the only freestanding mental health facility in Northern VA, is seeking an experienced Mental Health Professional II - Adolescent IOP to support our Intensive Outpatient Program. In this key role, you will be responsible for high quality, patient-centered care by providing supervised mental health services to patients and their families. The Mental Health Professional II - Adolescent IOP performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family and group therapy.

What You'll Do

• Conducts individual, group and family psycho-educational and process groups.

• Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.

• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.

• Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).

• Completes assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.

• Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge.

• Conducts post-discharge follow-up as indicated.

• Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.

• Develops and maintains a community resource guide including access to public assistance/alternatives.

• Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.

• Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities/resources.

• Identifies cultural considerations, gender identity and preferred language for patients and family.

• Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.

• Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers.

• Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.

• Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.

• Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.

• Demonstrates knowledge and role in hospital codes.

• Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services.

• Reviews, coaches, and assists with training of new staff/interns as assigned within approved practice and program guidelines.

• Participates in developing department goals/objectives and clinical programming.

• Performs utilization review as assigned.

• Assists with Process and Quality Improvement initiatives.

• Performs other duties as assigned.

Qualifications

Education: Master’s Degree in Social Work or related field-Required

Licensure: Currently registered as a Licensed Master in Social Work (LMSW) in the state(s) of practice, according to law and regulation.-Preferred. Currently licensed as a Licensed Professional Counselor Associate (LPCA)-Preferred

Certifications: Basic Life Support for Healthcare Providers required and Crisis Prevention Institute required as a part of the general hospital orientation.

Experience: Experience in Mental Health or related field-Preferred

Compensation for the role will depend on several factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
