Addus HomeCare, Inc. seeks a Home Care Service Coordinator to support quality in-home care for seniors and vulnerable adults. In this role, you will coordinate and schedule home care services, match caregivers to client needs, and serve as a key contact for clients, families, and field staff. You will ensure accurate documentation, compliance with regulations, and timely communication of changes in client condition. Working in a mission-driven, patient-focused environment, you’ll collaborate with clinical and administrative teams and gain opportunities for training and advancement in community-based care.
