Physical Therapist – SNF Facility (CA 95129) (Contract)
Schedule:
13-week contract (ASAP start)
36 hours/week
Travel or Permanent option available
Pay:
$95/hour
Position Summary & Responsibilities:
Evaluate, treat, and manage SNF patients
Develop and implement individualized PT treatment plans
Provide therapeutic exercise, mobility, and gait training
Document evaluations, progress, and outcomes
Collaborate with interdisciplinary care team
Support functional recovery and discharge planning
