Under the direction of the Operations officer , the Quality Improvement / Quality Assurance (QI/QA) Coordinator supports the development, implementation, and evaluation of the health center’s overall QA/QI and risk management program across ancillary, nursing, and medical staff departments to identify trends, prioritize and recommend improvements, decrease duplication, and ensure regulatory compliance. The Coordinator may provide training, technical assistance, and support services to help establish, promote, maintain, and enhance Quality Improvement and patient safety systems. Such activities may include training clinic staff; monitoring program effectiveness; assisting in preparation for accreditation surveys; conducting audits and studies; identifying and evaluating organizational risk; and maintaining an effective, outcome-driven Quality Improvement and Risk Management program.

Schedule:

5 days per week, 8 hours per day

Monday - Friday (with rotating Saturdays as needed)

Shifts:

8:00 – 5:00 PM, 9:00 – 6:00 PM, and 10:00 – 7:00 PM

Locations:

San Bernardino and Banning, CA

RESPONSIBILITIES AND DUTIES

Typical responsibilities and duties include, but are not limited to:

• Work with medical staff department leadership on QA/QI activities.
• Organize, compile, and analyze QA/QI data to identify trends, establish priorities, and recommend improvement activities.
• Collect and review data to identify gaps or weaknesses in the health center’s procedures and recommend improvements.
• Review clinical policies and procedures; ensure compliance and recommend modifications based on identified needs and trends.
• Perform comprehensive, concurrent, and retrospective reviews of operational data using criteria developed and approved by the health center.
• Participate in and complete customer service audits.
• Assist medical providers and scribes with documentation needs and/or training so that patient health record documentation meets or exceeds standards.
• Participate in the Utilization Management system by reviewing routine and critical labs and scheduling patients for results.
• Review urgent and routine referrals and assist with patient follow-up.
• Review the hospitalization/transfer log and conduct patient follow-up related to hospital visits.
• Track project progress and communicate findings and outcomes in a timely manner.
• Report findings to other departments and/or appropriate committees on utilization review, infection control, and risk management.
• Support performance improvement, attainment of quality standards, and risk-reduction activities.
• Proactively identify and evaluate internal and external risks; conduct or support risk assessments in accordance with internal policies and governance.
• Participate in monitoring risk areas across departments and sites to support consistent application of internal risk processes and standards.
• Maintain the incident/occurrence reporting system; evaluate incidents for severity, frequency, and probability of loss, and report to appropriate administration and committees.
• Draft and submit incident and accident reports to the appropriate persons and agencies, and maintain related documentation confidentially; may be consulted on potential claims as needed.
• Support the development and tracking of corrective action plans to mitigate identified risks.
• Develop reports on quality and risk trends and communicate findings to appropriate departments and committees, including the QI Committee.
• Serve as a resource to staff on quality, risk management, and patient safety; address situations involving potentially compensable events with discretion and professionalism.
• Contribute to the development of annual quality and risk management goals and objectives.
• Other duties as directed by Supervisor and/or Administration.

MINIMUM QUALIFICATIONS

• High School Diploma or GED required.
• Bachelor’s degree required. (Science, Healthcare Administration, Business, or related field)
• Three (3) to four (4) years of related experience in quality improvement, quality assurance, risk management, or a clinical/healthcare setting.
• Must have experience with an electronic health record (EHR) system in a medical setting; experience in a Doctor’s office or community health center preferred.
• Working knowledge of local, state, and federal regulations regarding healthcare operations preferred.
• Must be able to accurately identify quality processes and efficiencies and work with the appropriate resource, policy, procedure, or team member.
• Must be able to recognize and escalate issues as needed.
• Bilingual English/Spanish preferred.
• Must be able to travel to all IBHS locations.

ESSENTIAL FUNCTIONS

• Must have physical ability to sit for extended periods of time, reach, possess fine manual dexterity, talk, and hear.
• Must be detail-oriented with effective written and verbal communication skills and proficiency with Microsoft Office (Word, Excel, PowerPoint).
• Must be psychologically able to deliver appropriate services to patients regardless of ethnicity, religion, sex, age, physical disability, or economic status.

UNIVERSAL REQUIREMENTS
