We are seeking a Case Manager to oversee the service planning and transitional process for youth entering and exiting the program. The ideal Case Manager is excited to develop individualized plans for each youth we serve while monitoring those plans to ensure effectiveness. What you will be doing: Complete intake needs assessment, re-assessments, and transition planning. Evaluate, develop and document an individual service/care plan for the youth based on the needs assessment, and complete required updates. Active participant in the Multidisciplinary Treatment Team (MDT). Deliver evidence-based treatment models and adhere to fidelity requirements, including documentation. Attend and represent the program at court hearings, meetings, and youths' legal proceedings. Role requirements: Bachelor’s degree in psychology, social work, or a related field.
