Job Description:

This role requires an experienced RN Case Manager with a focus on acute inpatient case management and discharge planning. Candidates must be able to work efficiently with minimal training and coordinate with a team of social workers to care for complex patients in a Level 1 trauma hospital.

Responsibilities:

• Manage a caseload of 15-20 patients per day across various inpatient units.
• Work collaboratively with a team of 15 full-time and 17 part-time social workers.
• Consult with Pharmacy and Respiratory Therapy for discharge planning as needed.
• Participate in a two-week department-specific orientation, including general facility orientation.

Requirements:

• Minimum 3 years of acute inpatient case management experience, including at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center).
• Prior travel experience in a Level 1 facility.
• Strong computer skills and experience with Epic EMR.
• Nursing degree (RN, BSN preferred, will consider ADN).
• BLS certification through AHA, CCM preferred.
• Recent acute discharge planning experience (within last two years).
