Remote RN Care Manager - Care Coordination & Transitions

ArchWell Health

ArchWell Health is seeking a Nurse Care Manager to coordinate care for patients with acute and chronic conditions across the care continuum. You will build care plans with the team and execute them to meet quality, regulatory, and patient outcomes goals, from our centers to post-acute and home environments. You will collaborate with clinicians, hospital staff, and community resources to optimize transitions of care, educate patients, and ensure a high-quality, person-centered experience for #J-18808-Ljbffr
