Full-Time Occupational Therapist (OT) – Skilled Nursing & Rehabilitation

Achieve Rehab & Nursing Facility is seeking a compassionate and dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Liberty, New York .

This is an excellent opportunity to work alongside a collaborative interdisciplinary team focused on helping residents regain independence, improve function, and enhance their overall quality of life.

• Welcoming environment for both experienced therapists and new graduates

• Evaluate residents and develop individualized occupational therapy treatment plans.

• Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents.

• Address activities of daily living (ADLs), upper extremity function, cognition, adaptive equipment needs, and functional mobility.

• Monitor resident progress and modify treatment plans based on clinical outcomes.

• Educate residents, families, and caregivers on therapeutic interventions, safety techniques, and adaptive strategies.

• Complete accurate and timely documentation using Net Health and PointClickCare (PCC) .

• Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes.

• Maintain compliance with all federal, state, and facility regulations.

• Current New York Occupational Therapist (OT) license or eligibility for licensure.

• Graduate of an accredited Occupational Therapy program.

• New graduates are encouraged to apply.

• Skilled Nursing Facility (SNF) or Long-Term Care experience is preferred but not required.

• Experience with Net Health and PointClickCare (PCC) is preferred.

• Strong communication, clinical reasoning, and organizational skills.

• Passion for providing compassionate, resident-centered care.
