Pharmacist - Full Time Nights

Pharmacist Valley Presbyterian Hospital Deliver Safe, Effective & High-Quality Pharmaceutical Care At Valley Presbyterian Hospital, our pharmacy team plays a critical role in patient safety and clinical outcomes. We’re seeking a skilled Pharmacist to provide comprehensive pharmaceutical care through clinical oversight, medication management, and collaboration with the healthcare team. What You’ll Do

- Review medication orders for accuracy, appropriateness, and safety
- Monitor drug therapy including dosing, interactions, and therapeutic outcomes
- Provide clinical interventions and collaborate with physicians on medication plans
- Interpret lab values and assess patient medication profiles
- Educate patients and healthcare staff on medication therapy
- Supervise pharmacy technicians and support staff
- Ensure accurate and timely dispensing of medications and IV preparations
- Monitor for and report adverse drug reactions and medication errors
- Support formulary management and non-formulary drug procurement
- Participate in performance improvement, safety, and compliance initiatives
- Strong clinical knowledge and understanding of pharmacotherapy
- Ability to assess, monitor, and adjust medication regimens
- Strong analytical and problem-solving skills
- Ability to collaborate effectively with interdisciplinary teams
- Strong communication and patient education skills
- Doctor of Pharmacy (PharmD)
- Current California Pharmacist license
- Ability to obtain LA City Fire Card within 30 days
- ASHP-accredited residency or equivalent clinical experience
- Experience with Meditech or similar systems
