This position provides an opportunity to work within a rural, community-based program serving patients throughout Virginia's Eastern Shore. The PTA will deliver high-quality therapy services to adult and geriatric patients in their homes.Responsibilities\n \n Collaborate with physical therapists and the interdisciplinary care team to implement tailored therapy plans.\n Provide skilled patient care, assisting patients in their recovery from illness, injury, surgery, and chronic disease.\n Focus on promoting patient independence, preventing hospitalizations, and enhancing overall quality of life.\n Manage and conduct approximately six patient visits per day within a geographically diverse service area.\n Document patient progress and treatment outcomes accurately and timely.\n Qualifications\n \n Current license or certification as a Physical Therapist Assistant.\n Experience in home health care is preferred, particularly working with elderly patients.\n Strong communication and interpersonal skills to build rapport with patients and their families.\n Ability to work independently and manage a flexible schedule.\n Willingness to travel to patients' homes across the designated service area.\n Work Schedule\n This position requires availability for day shifts, working five days a week, with eight-hour shifts from 8:00 AM to 4:30 PM.Additional Information\n Mileage will be reimbursed at the IRS rate for travel between patient locations.\n \n Wellspring Nurse Source Job ID #37967534.
