ELECT Case Manager
The ELECT program assists expecting, custodial, and non-custodial young parents in obtaining a high school diploma or equivalent. ELECT also supports participants in securing post-graduation employment, education, or training which promote skills as parents. The ELECT Case Manager will provide intensive individualized support of enrolled students who are eligible for the ELECT/EFI program in Lycoming, Bradford, Sullivan, and Tioga counties. Liaise with community service providers, educators, and health care providers for program participants. Provide individual and group educational sessions during the academic year and the summer months as outlined within the ELECT/EFI grant through the PA Department of Education.
Qualifications/Certifications
Associate degree in Education, Human Services, Social Work or related field and 4 years related professional experience. (Minimum)
Bachelor degree in Education, Human Services, Social Work or related field with 2 years related professional experience. (Desired)
Essential Duties and Responsibilities:
- Develop and provide group and individual sessions for enrolled students
- Promote the ELECT program through the development of community connections
- Establish and maintain necessary working relationships with community organizations to best serve students through education and appropriate referrals.
- Advocate for the rights, strengths, and needs of the participants
- Provide comprehensive student assessment and develop individualized plans for program participants to remain in school and find employment or enrollment in post-secondary education upon graduation.
- Evaluate, adapt, and/or create appropriate educational materials and services relevant to sexual, reproductive, preventative health, and parenting needs of the participants of the program.
- Encourage student self-sufficiency in learning to identify, access, and utilize resources effectively.
- Document all case management activities and educational items in the appropriate case files in a timely manner
- Ensure effectiveness of services in support of the participants
- Attend ELECT/EFI Annual Conference.
- Attend and participate in weekly staff meetings, County Assistance Office meetings, community organizations meetings when applicable, planning and development sessions, continuing education workshops, and conferences, as required. Superior interpersonal skills necessary to deal effectively, empathetically and courteously with students and their families, school district personnel, community agencies, and health care providers.
- Possession of all required clearances to be able to work with students under the age of 18.

- Functional ability to manage required data for measured outcomes related to the grant requirements.
- Ability to operate in a fast-paced, multi-project environment.
- Possession of a valid driver's license.

Special Position Features:
*Person will be required to use personal vehicle to transport students.
*Will be required to work occasional evenings.
*This position is subject to a background check and all required clearances.
