Case Manager

The Case Manager will report directly to the Executive Director of JFF. Key responsibilities include: Screening of perspective resident information to determine eligibility and resident needs. Supports the coordination of resident intakes and referral process. Develops individual service plans for residents and family - case management. Develops transition plans for residents completing the program Documents activities in resident records in accordance with JFF standards Provides supervision, protection, and care of residents individually and in groups. Documents residents progress; identifies possible need for professional services; communicates such findings to professional staff. Supports refinement of established policies, process, and implementation. Conducts mandatory trainings, in-service, and meetings, when necessary. Maintains confidentiality as required licensing, HIPPA, and other regulatory agencies. Develops relationships with other community-based organizations. Qualifications: Prior case management experience required. Previous experience working with youth and young adults required. Must be detail oriented. Ability to use good judgment in decision-making. Must be able to self-manage. Ability to independently use a computer (Excel, Word, email etc..) and the ability to learn necessary computer programs.
