Position Overview

Next Horizon Health Care is seeking a dedicated Nurse Practitioner (NP) to join our dynamic team in El Paso, Texas. Work Environment

You will work onsite at our outpatient office in El Paso, Texas, where you will be part of a collaborative care team that includes physicians, physician assistants, medical assistants, and dedicated medical scribes. Our team dynamic enhances the caregiving experience, while support from registered nurses and care managers allows you to focus on providing direct patient care. Key Responsibilities

• Deliver outstanding primary care, addressing both routine and acute health issues through thorough office visits.
• Conduct annual wellness visits and health risk assessments that focus on holistic health and precise documentation to enhance patient care.
• Facilitate care coordination with various providers, specialists, testing facilities, and agencies to streamline patient care.
• Lead population health initiatives by ensuring critical screenings and assessments are administered to eligible patients.
• Assist the care team with phone triage and outreach, promoting proactive health management among our patient population.
• Educate patients about their health conditions, care plans, and recommended treatments, ensuring they feel heard and informed.
• Engage in promotional activities to enhance community awareness of available services.
• Conduct home visits as needed to provide care for patients who cannot visit the office.
• Complete other responsibilities as assigned by the Medical Director, while closely collaborating with operational leadership.

Patient Population / Client Focus

Your primary focus will be older adults on Medicare, with an emphasis on providing compassionate, individualized care. By working collaboratively within our practice, you will have the opportunity to build long-term relationships with patients, guiding them in their health journeys.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and DEA license.
• Proven experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A collaborative spirit with a passion for team-based care and innovative health solutions.
• Bilingual proficiency in relevant languages is highly desirable.

Preferred Experience

• Prior experience working with Medicare populations or in outpatient settings is a plus.
• Demonstrated ability to engage with interdisciplinary teams to enhance patient care.
• Experience with electronic health record systems and documentation requirements.

Compensation and Benefits

This full-time position offers a competitive salary range between USD 120,000 to 160,000 per year, commensurate with experience and qualifications. Our benefits package includes:
